Physical Examinations
Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 14 school days prior to the first day of practice/tryouts.
Physicals are valid for 1 year from date of the last exam.
- Health History Update Questionnaire Form – must be filled out by the patient and parent when a physical is currently on file/within the 365 days of the 1st day of practice. Hard copy must be handed in to Nurse Smith.
- PPE History Form- Must be filled out by the healthcare provider completing the physical exam. This form is to accompany the PPE Exam Form.
- PPE Athletics with Disabilities – for a student with a disability that limits major life activities.
- Physical Examination Form/PPE Exam Form - be completed ONLY by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) that has completed the Student-Athlete
Cardiac Assessment professional development module; must be conducted within 365 days prior to the first official practice in an athletic season.
Exam form stays with home physician
- PPE Medical Eligibility Form - This form is to be submitted to the licensed physician, APN or PA which will be completed and sent back to the school upon medical clearance.
- Asthma Treatment Form – needs to be completed by your child’s doctor & parent. Hard copy form to be handed in to the nurse. Inhaler is expected to be with the student for ALL practices and games.
- Food Allergy Form – needs to be completed by your child’s doctor & parent. Hard copy form to be handed in to nurse, along with Epipen which will be placed n the Sports Medkit.
- Random Drug Testing Form (RDT) – to be completed by student AND parent / guardian.
- Opioid Use and Misuse Fact Sheet – to be read in its entirety prior to completing sign-off sheet.
- Opioid Use and Misuse Sign-off Sheet – to be signed and submitted in hard copy form only.
